Clarksville-Montgomery County School System say 7th Graders must have Immunization for 2018-2019 School Year

Clarksville Montgomery County School System (CMCSS)Clarksville, TN – The Clarksville-Montgomery County School System (CMCSS) says that all students entering 7th grade for the 2018-2019 school year must have a Tetanus-diptheria-pertusis booster shot (also known as Tdap).

7th grade students for the 2018-2019 school year must havee their immunizations and documented on the Official Tennessee Certificate of Immunization form.

Proof of this immunization must be documented on the Official Tennessee Certificate of Immunization form and this form must be turned into the school.

Please make arrangements with your healthcare provider or the health department for your child to receive this new immunization prior to the first day of the 2018-2019 school year.

It is state law that no child be permitted to attend school until proof of this immunization is provided.
